  1. Web/App Opt-in, Subscription
    1. Your consent to receive SMS messages from Pulmonx Corporation is initiated via a checkbox on our web form at the time of subscription. By checking this box, you agree to receive a confirmation text message from us. To complete the opt-in process and begin receiving text messages according to the terms outlined here, you must reply ‘YES’ to the confirmation text. Failure to reply ‘YES’ will result in you not being subscribed to the SMS service.
    2. When you opt-in to the service, we will send you a message asking you to confirm your signup. Text YES to 20526 to receive Zephyr® Valve Alerts. Message and data rates may apply. Message frequency will vary based on your use. Text “HELP” for help. Text “STOP” to cancel your subscription and no longer receive messages from us. If you want to join again, just sign up as you did the first time and we will start sending messages to you again.
    3. By opting into Pulmonx’s SMS service, you agree to receive periodic text messages about our products or health-related information tailored to your interests. The text messages are not intended to be a substitute for professional medical advice or to provide a medical diagnosis.
  2. In addition to your opt-in mobile phone number, we may collect other personal data, including name, , email address, zip code and birth date, to provide you with information related to our products, as well as for internal research and to perform user experience improvements.

We retain your data for as long as necessary to provide you with the information you have requested or until you choose to opt-out of our services, or for other essential purposes such as complying with our send you information.   4. You can cancel this service at any time. Just text “STOP” to 20526. After you send the message “STOP” to 20526, we will send you a reply message to confirm that you have been unsubscribed. After this, you will no longer receive messages from us.
  5. If at any time you forget what keywords are supported, just text “HELP” to 20526. After you send the message “HELP” to 20526, we will respond with instructions on how to use our service as well as how to unsubscribe.
  6. T-Mobile is not liable for delayed or undelivered messages.
